Evaluate patient referrals to the City, completing a comprehensive physical assessment, a review of functional status and elicits the goals of the patient and family to assist the team in determining the rehabilitation potential of the patient. Educates family and patient about rehabilitation process, the nurse liaison will interact with all the referral sources and with many professional organizations through the process. Nurse liaisons are the beginning of the continuum of care initiating the rehabilitation journey. |
